Report Storm Damages

The West Virginia Emergency Management Division (WVEMD) is working with your local/county emergency management agency to collect information on damage. Please fill out the survey (or scan the QR code below) so that emergency management officials have a better understanding of how severe weather impacted your community.

If you need help completing your survey, please call 211 between 8AM and 4PM.

Once the survey has been submitted, the information you provided will be sent to the WVEMD and shared with local/county emergency management officials.
Please submit only one survey per household. WVEMD and local/county emergency management will reach out to you if any additional information is needed.
If you have neighbors in your community that may have issues accessing the survey, please assist them by completing the survey on their behalf.
This survey is not an application for financial assistance. If financial assistance does become available, you will be notified via the contact information that you have provided.
If there is damage related to public infrastructure such as: fire departments, police stations, libraries, government owned bridges, or roads- please report them to your local emergency management agency or WVPA@wv.gov
